Family Midwifery is Out-of-Network
for all commercial and state insurance carriers and plans.
Our practice is registered with Tricare, and our services may be covered,
depending on your plan.

Some health shares and healthcare ministries do cover our services.

Or, if you’d like to contact your insurance carrier yourself, here are a few helpful questions:

Does your plan have "Out-of-Network" benefits? If so, what are they?

If not, can you apply for an “Out-of-Network exception”?

Does your plan cover "Out-of-Hospital Birth?" Ask if it covers both birth centers and home birth.

Does your plan cover services by midwives? Ask if it covers both CNMs (Certified Nurse-Midwives) and CPMs (Certified Professional Midwives).

Does your plan require pre-authorization for  services?

What is the time limit for submitting a claim for Out-of-Network services?
